How do I log in to the portal?
Go to the Oaktree Connect portal and sign in using the same email address you used for your registration with us on https://patient.oaktreeconnect.co.uk/signin
What should I do if I forgot my login email?
Please use the email address where you received your welcome or registration confirmation. If you still can’t access your account, contact our support team for assistance.
Documentation & Requirements
What kind of Photo ID do I need to provide?
We accept a valid Passport, Driving License, police or army ID card with photos. You can upload this directly to your portal or send it to us via email.
Where can I find the questionnaires, I need to fill out?
Once logged into the portal, look for the green column on the left-hand menu. You will find your assigned forms under the “My Therapy Tasks” section.
When do I need to complete my questionnaires?
Please ensure all assigned questionnaires are completed within the portal before your scheduled appointment with your practitioner.

What happens during the personalized consultation?
Your practitioner will spend time getting to know you, discussing your current issues, your personal history, and what you hope to achieve through your goals and needs.
What can I expect after my first assessment?
You will end the session with a clear path forward. This may include scheduling follow-up appointments, beginning a specific treatment plan, or receiving helpful resources to take home.

We are on Portland Road off the Hagley Road in Edgbaston, Birmingham, close to the city centre. The clinic is inside a red brick front House on the main road itself. The parking is for staff only but there is plenty of off-street parking around the clinic.
From the Grand central Station, West Midlands Metro to Edgbaston Village and from there it is a 15 minute walk to the clinic or a 2 minute taxi ride.
